Canada Dental Care Plan (CDCP) Policy

The aim of the Canada Dental Care Plan (CDCP) is to help make the cost of dental care more affordable for Canadian residents who do not currently have access to dental insurance. It is not a free dental care plan. As a CDCP client at our office, you may have to pay additional charges, in addition to any co-payment to our office that the CDCP does not cover.

Below are our office policies for the Canada Dental Care Plan. Please read them carefully before proceeding.

We require a minimum of 24 hours notice to cancel or change any appointment. A fee will be charged if an appointment is missed or cancelled without sufficient notice.

Your coverage must be in effect at the time of your appointment.

The CDCP does not pay the full cost of treatment. Sun Life will reimburse our office directly for eligible expenses. You are responsible for paying, directly to our office, any applicable co-payment amounts not covered by the CDCP at the time of your appointment.

Not all dental services are covered under the CDCP. Our providers will discuss the best treatment options for you based on your specific needs.

All fees charged are those set out by the College of Dental Surgeons of Saskatchewan. Our office is not following the CDCP fee schedule.

There is no coordination of benefits through other insurers, including Supplementary Health and Family Dental Benefits or private and employer plans.

Our office has the right to revoke participation in the CDCP at any time based on our discretion.

Credit Card on File

Optional. If you would like to keep a credit card on file for any patient portion of treatment cost, please enter it below.

Card details are entered directly into Moneris' secure tokenization form below. We never see or store your card number on our website.

Consent

I, the undersigned, consent to the performing of the dental and oral surgery procedures agreed to be necessary or advisable, including the use of local anesthetic as indicated, and will assume responsibility for fees associated with these procedures. I have read and agree with the policies of Cumberland Dental.
